In This Issue In This Issue Saying NO to muscular dystrophy ccording to the prevailing model for Duchenne muscular dystrophy (DMD) pathogenesis, a lack of dystrophin protein makes muscle cells susceptible to mechanical damage, leading to muscle breakdown. On page 123, Wehling et al. suggest that the major damage in DMD may actually be caused by a secondary consequence of dystrophin loss: destruction of muscle tissue by a patient's own macrophages. Dystrophin forms a complex with several other proteins, including nitric oxide synthase (NOS). In dystrophin-deficient muscles, such as those of the mdx mouse, a model for DMD, NOS expression is decreased. Reasoning that the loss of nitric oxide's anti-inflammatory activity might exacerbate muscle breakdown, the authors introduced a transgene to A Muscle cells experience a nuclear buildup n response to stimulation, muscle fibers can switch between two types: fast-twitch, used for quick movement; and slow-twitch, which are more resistant to fatigue. On page 27, Liu et al. find that the nuclear localization of a transcription factor may be the key to this switch. I Liu et al. applied electrical pulses to isolated adult murine muscle fibers, thus simulating fast-twitch and slow-twitch muscle stimulation in vitro. They then looked at the localization of the transcription factor NFATc, which has been implicated in muscle-and T-cell transcriptional regulation. NFATc is cytoplasmic in unstimulated fast-twitch muscle fibers, but Slow stimulation sends NFATc into the nucleus. translocates to distinct nuclear foci when the fibers are exposed to kinase inhibitors or trains of electrical pulses at 10 Hz, simulating slow-twitch stimulation. Two conditions do not cause NFATc nuclear-translocation: simulated fast-twitch stimulation, and continuous 1 Hz stimulation, which provides the same number of electrical jolts per minute as the 10 Hz pulses without mimicking any natural stimulation pattern. The nuclear NFATc localizes to distinct intranuclear foci. Liu et al. report that the foci are similar in size and shape to the Cajal bodies that are believed to be involved in splicing in certain cells, but the two structures may not be the same. The authors are now trying to identify additional components of the NFATc-containing foci, and hope to use their in vitro system to determine whether NFATc translocation alone is sufficient to initiate the conversion of fast-twitch muscle fibers into slow-twitch fibers. produce normal levels of NOS in the muscles of mdx mice. The transgene reduced the concentrations of macrophages in the muscles and …

Volar Locking Plate Breakage after Nonunion of a Distal Radius Osteotomy
We report a 38-year-old male with a nonunion followed by plate breakage after volar plating of a distal radius osteotomy. Volar locking plates have added a new approach to the treatment of distal radius malunions, due to a lower morbidity of the surgical approach and the strength of the final construction, allowing early mobilization and return to function. Conclusion. Plate breakage is an unco...
متن کاملSevere intrauterine growth retardation with increased mitomycin C sensitivity: a further chromosome breakage syndrome.
We report an infant with pre- and postnatal microcephaly and growth retardation, a distinctive face, and developmental delay. The initial diagnosis was of Seckel syndrome. He became pancytopenic at 16 months and died soon after. His bone marrow was of normal cellularity but had a small lymphocyte infiltration. Increased spontaneous chromosome breakage was seen in blood and fibroblasts. Mitomyci...
متن کاملBiomechanical effects of bone-implant fitness and screw breakage on the stability and stress performance of the nonstemmed hip system.
BACKGROUND Some nonstemmed hip systems have been developed to avoid stress shielding and aseptic loosening, which are major drawbacks of stemmed hip arthroplasty. Without the stem, the cup over the femoral head can be stabilized by anatomic fitness of the cup interior and mechanical fixation of the auxiliary screws. METHODS Using finite-element method, neck-shaped systems with two bone-cup fi...

The purpose of this study was to investigate cement leakage (CL) in osteoporotic vertebral compression fractures (OVCFs) with cortical defect using high-viscosity bone cement during unilateral percutaneous kyphoplasty (PKP) surgery. This study included a series of 77 patients (23 males, 54 females) with single level osteoporotic vertebral body fracture (OVCF) who underwent unilateral PKP in our ...
متن کاملRevision surgery after pregnancy in a patient with congenital kyphoscoliosis
RATIONALE Rod breakage during pregnancy and delivery has never been described in a patient who has undergone surgery for congenital scoliosis (CS). Here, we present an unusual but significant case of revision surgery. PATIENT CONCERNS A 29-year-old woman presented with low back pain during pregnancy after posterior osteotomy, correction and fusion at T9 to L5 for CS. Radiographs during follow...
متن کاملSymptoms Indicating Imminent Breakage of a Femoral Interlocking Nail: A Case Report
UNLABELLED Fractures of the femoral shaft treated with interlocking nails will ultimately result in breakage of the nail if the bone does not unite. Further management requires removal of the broken nail which may be a difficult process for the distal segment. If we can identify the symptoms just before the nail breaks, an exchange nailing becomes much easier. We present a patient with fibromat...
